(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1996-008 <br />INSPECTION DATE 7/2/03 <br />INSPECTOR'S INITIALS GRM <br />This inspection was preformed as part of the normal monitoring program established by the Colorado Division of Minerals and <br />Geology. The North R-34 Pit is located just north of LaSalle Road, west of the Western Gravel Main Plant Site. The 112c <br />permitted site is active mostly during the winter months. Milan Hauxby, the operator s representative was present for the <br />inspection. <br />The mine identification sign and affected area boundary markers are in place and in compliance with Rule 3.1.12. The main <br />sign is located at the entrance off of LaSalle Road. Permit markers include fence lines and roads that make the permitted area <br />easy to distinguish. <br />The operation is still in Phase 1. Approximately 5 acres is excavated with approximately 1 acre to a depth of 40 feet. The <br />ground water level in this area keeps approximately'/. of the area covered most of the year. The application for this permit <br />states that a well permit was applied for expose ground water. The Division s permit file does not have a copy of the well permit <br />application or approval. Since around water is exoosed. staff requests a coov of the aoProved well uermit. This issue <br />is listed as a oroblem with corrective actions and correction dates noted at the end of this report. If a well permit was not <br />issued, a problem or possible violation may exist to the hydrological balance and staff will have to review the issue accordingly. <br />The pit access road is in good repair. No erosion or sedimentation issues were observed. All storm water appears to be <br />maintained on site. <br />Highwalls are 10 to 15 feet in depth with an excavation point that is approximately 30 deeper, which is under water presently. <br />Topsoil stockpiles were observed along the western edge of the pit. At this time the material appears to be adequate for the <br />required sloping during reclamation. <br />No significant infestations of noxious or other problematic weeds were noted on site during this inspection. <br />The Financial Warranty was reviewed in 2001 by Specialist Bob Oswald. Current conditions reflectthose taken into account at <br />that time. Therefore, the Financial Warranty appears to be adequate at this time.
