_~ <br /> <br /> <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-80-244 <br />INSPECTION DATE: 4-21-00 INSPECTOR'S INITIALS: ~_ <br />OBSERVATIONS <br />Conducted a monitorin¢ inspection of the site on 4-21-00. The main purpose of the inspection was to collect water sample <br />for WAD CYANIDE analyses from the North Underdrain AKA South West Underdrain and the South Underdrain, AKA <br />South East Underdrain. Present durin¢ the inspection were Phil Barnes and Lawrence Myers of CC&V. Inspected the <br />underdrain dischar¢e area. Both 24-inch perforated pipes were dischar¢in¢ at the time of the inspection. The dischar¢e from <br />the North underdrain was measured at 10¢pm while dischar¢e from the South Underdrain was measured at 15¢pm. Liter <br />samples from the North and South underdrains were collected with a split to the operator .Inspected the Phase III area, <br />backfill and ¢radin¢ of the slopes in anticipation of soil liner fill placement were in pro¢ress. Ore and drain cover fill were <br />bein¢ removed from the toe of the West facin¢ slopes of the Phase II teach pad area where liner will be extended on to the <br />Phase III area. As expected, there were some liner tear noted, which will be repaired prior [o extendin¢ the liner. At the <br />time of the inspection, there were no Golder OA/OC personnel on site. The approved OA/OC specification requires a <br />Golder OA/OC personnel to be present durin¢ such critical operations. Mr. Phil Barnes of CC&V was notified of the possible <br />problem. He explained that he was sure Golder QA/OC personnel were present in the area but he did not know where they <br />were at that particular moment. He said, he will talk to the resident en¢ineer for the construction phase and will ¢et back to <br />me prior to my departure from the area. However, even if the OAIOC individual from Golder was not at that area, there <br />was indeed an Ames laborer ¢uidin¢ the excavator operator so as not to dama¢e the liner more than what was accepted <br />when undertakin¢ such a removal operation from the liner. It was also noted that on the already exposed liner, from the <br />same operation, liner defects were marked usin¢ a white chalk. After the inspection and after I observed the 1.000,000 <br />ounces of Gold pour. 1 went to the mine office and talked to Mr. Phil Barnes. He stated, after talkin¢ to the resident <br />en¢ineer for the site he was assured that Golder personnel were on site, they iust happened to be in another area, <br />On 4-24-00, Mr.Scott Lewis Man¢er Environmental Affairs contacted the Division to express the fact that the approved <br />QA/OC specification for the site only requires Golder OA/OC personnel to be present only when drain cover fill is placed. <br />He further explained under such conditions, the Division's inspection report should not indicate a problem.
