(Page '2 ) <br />MINE ID # OR PROSPECTING ID # M-1983-194 <br />INSPECTION DATE 05/21/07 <br />OBSERVATIONS <br />INSPECTOR'S INITIALS ESC <br />This was an inspection of the Nahcolite'Project site conducted by Erica Crosby of the <br />Division of Reclamation, Mining and Safety. Cindi Eyrns of Natural Soda, Inc. was present <br />during the inspection. <br />The mining and reclamation permit was issued to Natural Soda, inc. on January 31, 1984. <br />The total permit area is 153.10 acres whereas 72 acres is to be affected at any given <br />time. The site is on land managed by the Bureau of Land Management (BLM). The Division <br />holds a bond in the amount of $150,750.00. The BLM holds the remainder of the bond. <br />The Nahcolite Project site is an active mining operation. Natural Soda recently submitted <br />and received approval for Technical Revision (TR-24) for a new vertical production well, <br />5H-lv, which will be completed in the existing 5H Cavity. The new well will replace the <br />existing lA recover well which has reached the end of its economic life and will be <br />plugged and abandoned in the near future. <br />A majority of the wells were inspected, and comments are as follows; <br />Well Status Coa~enta <br />4-3H Plugged & Abandoned The area is reclaimed and perennial <br /> grasses are well established consisting of <br /> Blue Flax, Alfalfa, Indian Ricegrass and <br /> Wheatgrass. <br />3A-4H Plugged & Abandoned The area is reclaimed and perennial <br /> grasses are well established consisting of <br /> Blue Flax, Alfalfa, Indian Ricegrass and <br /> Wheatgrass. Some bare spots were noted <br /> and will need to be revegetated. <br />DS-2 Monitoring Well- up In March/April 2007 a day logger was <br /> gradient installed for the well. <br />BG-1 Monitoring Well- up In March/April 2007 a day logger was <br /> gradient installed for the well. <br />3A-5V Pad area needs to be reclaimed <br />lA-SHA Active Production Well The well is currently in production. This <br /> well will shut down once 5H-1v is <br /> operational. Mud pits are still in place. <br />2B-3C Plugged & Abandoned Area has been reclaimed. The backfilling <br /> consists of small round piles of dirt. <br /> Vegetation has established in the dips or <br /> valleys of the piles but not on the top. <br />4A-5A Monitoring Well TDR cable installed in this well but is <br /> failing. Mud pit will need to be <br /> reclaimed. Area has been revs stated. <br />4A-4V Production Well The well is not producing and needs to be <br /> plugged and abandoned. The mud pits will <br /> need to be backfilled, area regraded, <br /> piping removed, and reseeded. <br />4A-1V Water level for the DS aquifer. <br />7H-1R Production Well Well drilled in 2005 but not completed to <br /> be functional. Will try to recover well <br /> by pulling the 3y, casing and perforating <br /> the 534 casing. <br />7H-2I Production Well Well drilled in 2005 but not completed to <br /> be functional. Will try to recover well <br /> by pulling the 3y, casing and perforating <br /> the 5'h casing. <br />SH-1V Production Well (TR -24) Pad area currently under construction. <br /> Mud pits have been installed and topsoil <br /> has been stockpiled along the perimeter of <br />
