<br />Part 3 -Support Facilities &Hdrologic Structures -To be completed at Permit Renewal and <br />Midterm <br />1. Verify that all roads are documented in the permit. If not, list all undocumented roads and their <br />approximate width and length. Verify the type of surfacing used on each road (gravel, asphalt <br />pavement, etc.) <br />All roads are documented in the permit. The main road to the site is asphalt to the yard. Other <br />internal roads are gravel. <br />2. Verify that all ditches and diversions are documented to the permit. If not, list their width, <br />depth, and length. <br />The ditches are documented in the permit. <br />3. Verify that all culverts are documented in the permit. If not, list their diameter and length. <br />The culvert is documented in the permit. <br />4. List the location, type of opening (drift, shaft, incline), and dimensions of all mine openings. <br />There are no openings. <br />S. Verify that all drill holes, boreholes, degas wells, etc., are documented in the permit. If not, list <br />the type of opening, its location, diameter, and depth. Document dimensions of any structures <br />associated with them. <br />All drill holes are documented in the permit. However, all the dimensions of those documented aze <br />not identified in the PAP. Those still open include: <br />FPW 1 (fire protection) and LFH #1 (water supply well), both of which aze registered with the State <br />Engineer; and monitoring wells DH 96, DH 122, SMW-2, AMW-1, and AMW-2. There aze no <br />structures associated with these wells. <br />6. List all off-site remote structures, their composition, and dimensions. (Example: ventilation fans, <br />weather stations, rail loops, loadouts) <br />Still in the permit azea, but remote from the main mine site, is the culvert on the access road. 50 feet <br />long, 96-inches in diameter. <br />7. List all sedimentation ponds and impoundments, and their locations. Document the type and <br />approximate dimensions of the structures. <br />Sediment pond 2 is located at coordinates E 40200 N 33700. It is mostly excavated with an earthen <br />berm, combined concrete spillway. It is approximately 400' x 250' x 5' deep.
