III. COMMENTS -COMPLIANCE <br />Below are comments on the inspection. The comments include discussion of observations made <br />during the inspection. Comments also describe any enforcement actions taken during the <br />inspection and the facts or evidence supporting the enforcement action. <br />needs to be reduced to about 3H:1 V. The operator has agreed to rework that portion of the slope. <br />There are some additional design aspects of pond C that do not conform with what is out in the <br />field. As shown on Map No. 22-C, the primary dewatering pipe should be on the southeast <br />portion of the pond. However, the primary was actually placed about 100 feet to the west. Also, <br />the design shows that the east portion of the pond embankment terminates into the hillslope. <br />However, in the field, the east portion of the pond embankment terminates at a large concrete <br />abutment, which is not shown on the map. Aproper as-built should be resubmitted, showing the <br />proper location of the primazy dischazge pipe, and the location of the concrete abutment. In <br />addition, the Division understands that the pond C embankment abuts directly against the <br />concrete structure, without any anti-seep device to act as an interface. The Division will be <br />requesting, in a letter, that the operator provide some kind of anti-seep device for this portion of <br />the pond embankment. In the sazne letter, the Division will be requesting a modified as-built <br />map, showing the pond B configuration as it exists in the field. <br />SIGNS AIVD MARKERS <br />Mine ID signs were present at the appropriate locations, and contained the correct information. <br />EXPLOSIVES <br />The operator has located the temporary explosives storage area on the east material storage area, <br />which is east of and adjacent to pond C. The two powder houses were locked. However, the <br />trailer containing the ANFO was not locked.
