Box 620490 <br />Littleton, CO 80162-0490 <br /> <br />RE: Leadville Mill - File No. M-1990-057, <br /> Amendment (AM-2) <br /> Leadville Mill 110(d) Amendment <br /> <br />Dear Nick Michael: <br /> <br />On December 29, 2025, the Division of Reclamation, Mining and Safety received your 110d Hard Rock <br />Reclamation Permit Amendment application for the Leadville Mill, which is located in Lake County. <br />Preliminary review of the information received determined that the following additional information must <br />be received before the Division can consider your application complete and technical review can begin. <br />Thus, your application is incomplete for the purposes of filing as of January 9, 2026. <br /> <br />1. Page One of the Application Package lists the currently permitted acreage at 9.9 with a reduction of 1.6 for <br />a total of 8.3 Acres however the Revision Intake form from the Division’s ePermit system states the current <br />permit acreage at 8 with an increase of .3 acres to make 8.3 Acres. Division records reflect that the currenlty <br />permitted acreage is 8.00. Please provide a replacement Page One of the application package with the correct <br />numbers. <br /> <br />2. Exhibit E was submitted with various maps and the cross sections portion of the Reclamation Plan, <br />however no site wide Reclamation Plan Map was submitted. Please provide a site wide Reclamation Map. <br /> <br />3. Photo documentation of the notice sign posted at the entrance to the site was provided, however the <br />required signed affidavit of posting the notice was not included. Please provide the required signed affidavit <br />of posting the Notice. <br /> <br />You have sixty (60) days from January 9, 2026 to submit all necessary documents that the Division needs for <br />the application to be considered filed. If, at the end of the sixty day period, the application has not been <br />determined to be complete and filed, the Division may deny the application and terminate the application file. <br /> <br />This letter shall not be construed to mean that there are no other technical deficiencies in your application. <br />The Division will review your application to determine whether it is adequate to meet the requirements of <br />the Act after submittal of all required items. <br /> <br />If you have any questions, please contact me by telephone at (303) 919-2997, or by email at <br />Lucas.west@state.co.us. <br />
