Preliminary review of the information received determined that the <br />following additional information must be received before the Division can consider your application <br />complete and technical review can begin. Thus, your application is incomplete for the purposes of filing as <br />of December 11, 2025. <br /> <br />• Per Rule 4.17.1 (2) –The Operator shall include in the notice to the Office the names and addresses and <br />phone numbers of all owners of record to the affected land. <br />• Provide an updated map which depicts the current permit boundary. Within that map delineate the areas to <br />remain vs areas requesting to be released. As well as state the acreages associated with each. <br /> <br />You have thirty (30) days from December 11, 2025 to submit all necessary documents that the Division needs <br />for the application to be considered filed.
