Table 6 - CQA Earthworks Testing Summary – Drain Cover Fill (Crushed Ore): The Specification – <br />Percent Passing column header should list the range for the 2” and 0.75” grain sizes identified in the <br />specification and as shown on Appendix G.3 data sheets. Please correct the table. <br /> <br />Appendices <br /> <br />13. Appendix A - Staff Schedule for CQA Monitor Personnel: On Page 1, Angel Villar was present on <br />certain dates as “LS” (lead soils technician) but fails to include this acronym on the Page 1 table foot <br />note. Please amend the footnote to include this title. <br /> <br />14. Appendix A - Staff Schedule for CQA Monitor Personnel: The deputy project certifying engineer <br />(DPCE), Scott Parker, is incorrectly labeled as the “SPCE” in the table on Page 1. Please either <br />justify this discrepancy or correct the table to reflect the correct title. <br /> <br />15. Appendix D - Soil Liner Fill Acceptance Forms: These SLF Acceptance Forms show sequential area <br />locations and size (e.g. 6/13/25: P-4962/4966 - 45,023 sq.ft.) however, the 7/9/2025 form has only <br />the starting panel listed and no square footage amount. Please clarify the reason for the missing data, <br />confirm that samples were included in this missing area, and revise the acceptance form accordingly. <br /> <br />16. Appendix D - Soil Liner Fill Acceptance Forms: The square footage measurements given on all SLF <br />acceptance forms add up to ~1,119,668 sq. ft. Section 2.5 of the Introduction states that ~1,180,000 <br />sq. ft. of 80-mil LLDPE geomembrane were placed within the VLF2 Phase 3 Stage B.1 area. It is the <br />Division’s understanding that SLF and geomembrane were placed across the whole of the Stage B.1 <br />area and that all areas of SLF were accompanied by a quality check and corresponding Acceptance <br />Form. Please clarify why square footage measurements from SLF acceptance forms do not equal the <br />amount of geomembrane placed in the Stage B.1 area. <br /> <br />17. Appendix G.3 - Drain Cover Fill (Ore) Laboratory Test Results: The lab results sample printout for <br />DCFO-1-C has a range of 47-100 for the .75” grain size while the specification has a range of 40- <br />100. Please explain the discrepancy and correct any errors. <br /> <br />18. Appendix I - 80-mil Geomembrane Installation Summaries: Besides Appendix I.8 - 80-mil <br />Geomembrane Acceptance Forms, Appendix I and the sub-appendices do not contain fly sheets <br />between each section. Please resubmit Appendix I with the appropriate fly sheets and, if <br />electronically, the PDF bookmarks for each section. <br /> <br />19. Appendix I.4 - Extrusion Weld Summary: Please explain why no destructive samples were collected <br />on June 25th and July 23rd as according to the specifications, a destructive sample is supposed to be <br />collected 1/500 feet of welding or 1 sample collected per day. <br /> <br />This concludes the Division’s Adequacy Review of TR153. This letter shall not be interpreted to mean that <br />there are no other technical deficiencies in your request as other issues may arise when additional <br />information is supplied. The decision date deadline for TR153 is set for November 19, 2025. If there are <br />outstanding issues that have not been adequately addressed prior to the end of the review period, and no <br />extension has been requested, the Division may deny this Technical Revision.
