5. Describe proposed surface excavation or other land disturbance, including roads, pits, <br /> trenches, waste piles, drill pads and collar areas of underground workings, ponds, etc. <br /> ONE TRAC HOE WILL DIG A 6X6 FOOT HOLE 25 ' DEEP. A 5 ' CULVERT <br /> WILL BE PLACED INSIDE THE HOLE AND THEN BACK—FILLED. 16 CU. YDS. <br /> WILL REMAIN ON THE SURFACE AND PLACED NEXT TO THE SHAFT. A BERM <br /> WILL ALSO BE CONSTRUCTED TO CONTAIN ANY RUNOFF. (SEE ENCLOSED DIAGRAM) <br /> AT NO TIME WILL THE HOLE BE LEFT OPEN. <br /> 6.
