NUVEMCO,, LLC. <br /> RECEIVED <br /> April 2, 2025 APR 1 2025 <br /> Dustin Czapla Colorado Division of Reclamation, <br /> Environmental Protection Specialist Mining and Safety <br /> Division of Reclamation, Mining and Safety <br /> RE: Blue Streak Mine, NOI #P-2007-057, NOI Modification (MD02) <br /> Blue Streak Mine, NOI#P-2007-057, NOI Modification (MD02) is only, proposing to add the 5 drill holes to the NOI.The <br /> application is not proposing to extract an additional 999 tons of material from the approved extraction of material as shown <br /> in the original 2007 NOI, <br /> Nuvemco acknowledges that drill holes MW1 and MW2 that will be converted to monitoring wells, and will be in <br /> compliance with Rule 5.4.4. <br /> Rule 5.4.4 Conversions to a Monitoring Well <br /> (a) has obtained the necessary permit from the State Engineer(Division of Water Resources); <br /> (b)cases and seals the drill hole in accordance with the requirements of the Water Well <br /> Construction; <br /> (c)caps the drill hole to prevent unauthorized entry and injury to persons and animals; and <br /> (d)submits to the Office a copy of the"Well Construction and Test Report"submitted to the Division <br /> of Water Resources describing the method and materials used in casing and sealing the drill hole to <br /> prevent commingling of aquifer waters <br /> Rule 2 CCR 402-2-16—Standard for Plugging, Sealing,and Abandoning Wells and Boreholes will be followed in the <br /> reclaiming of drill holes MW3, MW4 and MW5.
