The 19H-1V well will be located on an existing pad. <br /> The 19H-IR-E will be drilled on a new pad to be located southeast of the existing 10H-13H <br /> production pad. This new pad location will be within an off-lease BLM Use Permit and the <br /> DRMS permit area and will be sited near an existing oil and gas access road. The pad will <br /> require a short, new access road, the pipeline for the 19H-IR-E will utilize as much existing <br /> disturbed ground up to the access road (see the attached map for the proposed pipeline route). <br /> The wells may be abandoned as follows: <br /> 19H-1V: A Cast Iron Bridge Plug (CIBP) will be placed at 2,200 feet MD GL and the 9.625-inch <br /> casing will be filled to surface with a minimum of 170.1 bbls of cement. <br /> 19H-IR-E: A CIBP will be paced at 2,050 feet MD GL and the 9.625-inch casing will be filled to <br /> surface with a minimum of 158.5 bbls of cement. <br /> Please find attached: <br /> 1) DRMS Form — Request for TR Cover Sheet <br /> 2) Location Map <br /> 3) Proposed Well Completion Diagrams <br /> 4) Plat, cut and fill diagrams and location photos <br /> 5) Check number 8167 in the amount of$1,006.00 <br /> Should you have any questions or comments, please contact me at(970)254-1224 or <br /> (970) 216-1010 (c), or Roy McClung at(970) 456-1289. <br /> Regards, <br /> 42ca— <br /> Gerald J.
