Fremont County approved a major modification to the Conditional Use Permit (CUP 12-003) on
February 25, 2020. This extended the exploration permit boundary to 3,172 acres. The proposed
monitoring wells are located within the CUP boundary.
Zephyr includes two maps as required in the NOI. Map 1 is the Location Map described in NOI
Item B.3. Map 2 is the proposed disturbance locations as described in NOI Item II. Monitoring well
locations and surface monitoring points are provided in Table 1. The monitoring wells are not only located
to comply with the approved monitoring plan and QAPP from May 23, 2022 but also to minimize the
environmental disturbance.
Table 1: Monitoring Location Coordinates and Elevations
Co State Plane
Central Zone
Location Description N E Elevation(ft.)
Compliance Well C 1205866 3060898 6124
Monitoring Well#1 MW-1 1205059 3059403 6259
Monitoring Well#2 MW-2 1204240 3058649 6368
Monitoring Well#3 MW-3 1203257 3058221 6501
Monitoring Well#4 MW-4 1204124 3056904 6577
Monitoring Well#5 MW-5 1203948 3055214 6776
Existing Monitoring Well WM-6 1203715 3058396 6438
Dawson Drainage Surface Point DDS 1203619 3058294 6445
Grape Creek Upper Surface Point UGC 1209310 3076623 5797
Grape Creek Lower Surface Point(LGC) 1216726 3066842 5384
Thank you in advance for your time and attention.
