Provide an equipment spotter equipped with a radio. X <br /> I. EQUIPMENT OPERATION <br /> Seatbelts shall be used at all times. X <br /> Equipment always has the right-of-way. X <br /> Safe parking procedures (chock blocks, etc.) shall be used at all times. X <br /> All lights, horns, brakes, wipers, back-up alarms and other safety X <br /> equipment shall be fully operational at all times. <br /> Each piece of equipment shall have at least one portable fire X <br /> extinguisher. <br /> All mobile equipment, with the exception of pickup trucks with X <br /> unrestricted rear view, will be equipped with back-up alarms. <br /> Mobile equipment shall be equipped with roll-over-protection (ROPS) <br /> and/or falling object protection structures (FOPS) as required by MSHA X <br /> regulations. <br /> All machinery shall be secured to ensure zero mechanical potential X <br /> prior to work being performed on the machinery. <br /> Equipment shall be maintained in safe condition. X <br /> Provide a driller's safety platform. N/A <br /> Inspect hydraulic and pneumatic lines and couplings daily. X <br /> Provide whip checks for all airline connections. N/A <br /> J. ELECTRICAL SAFETY <br /> Electrical lock and tag procedures are to be used at all times. N/A <br /> Prepare a plan for working in the vicinity of overhead electrical N/A <br /> transmission lines. <br /> K. SAFE WORK IN ELEVATED AREAS <br /> Fall Protection shall be a full body harness and shock-absorbing lanyard. N/A <br /> Wire rope lanyards are required for cutting and welding operations. N/A <br /> All personnel are to be tied off if working where a fall hazard exists. N/A <br /> Ladder work procedures shall include set up, tie off, inspection and <br /> protection from overhead electrical hazards. Only non-conductive N/A <br /> ladders are to be used. <br /> L.
