f,OLORADO HESTMORELANO INC. <br />COLORADO MINED LAND RECLAMATION BOARD CERTIFICATION <br />SEDIMENTATION PONDS 1-4, <br />TIMBER STORAGE, STOCKPILE & LOA DO UT PONDS <br />PROFESSIONAL QUALIFICATIONS <br />Matthew S. Sakurada graduated from Colorado State University with a Bachelor of <br />Science-Civil Engineering in 1974, He also graduated from the Iniversity of <br />Colorado with a Master of Engineering degree in 1979. He has been a profession- <br />al engineer in the State of Colorado since 197R, He has experience and knowl- <br />edge in the design and construction of dams, drainage control, hydrology, and <br />structures, He is also experienced in permitting of coal mine facilities and <br />permit compliance. <br />RULE REQUIRING CERTIFICATION <br />This certification is required by Rule 4,05,6(7) of the Regulations of the Colo- <br />rado Mined Land Reclamation Board for Coal Mining, <br />SUBJECT OF CERTIFICATION <br />This certification is for mine site sedimentation ponds ~1, 2, 3 and a, and <br />storage, stockpile and loadout sedimentation ponds, <br />ACTIONS TAKEN TO PREPARE CERTIFICATION <br />The design of the Din wall sedimentation ponds was completed by Charles Leaf, <br />P.E. Calculations were completed by Matthew S. Sakurada on the stability of the <br />dams after completion as required by CMLRD rules, <br />Laboratory tests were taken to classify soils in the area and complete a Proctor <br />test for compaction purposes, <br />Field on-site inspections were completed daily from October 30, 1978, through <br />P+ay 25,•1979. Compaction, assembly, culvert installation, and other field con- <br />struction was inspected and documented, <br />The construction followed the documents submitted in permit application X77-353 <br />to the Colorado Mined Land Reclamation Division and the Bureau of LanA Manace- <br />ment right-of-way document C-25150. <br />Matthew S. Sakurada consulted with Charles Leaf, P.E., nesigner of the sedimen- <br />tation dams; John Fenner, Sales Engineer, ARMCO Steel Corporation, on the in- <br />stallation of the bin walls; Edward M. Morris, Lincoln-Oevore, was consulted er <br />compaction requirements. <br />PROFESSIONAL OBSERVATIONS AND CONCLUSIONS <br />All ARMCO assembly instructions were followed in construction of the him walls, <br />Compaction was completed utilizing pnrtahle "jumping jacks" as specified in =- <br />to B-inch free of rock layers. The culvert was installed per ARMCO specifica- <br />tions. The actual volume of the pond was field surveyed by CWI personnel. Thz <br />sediment clean-out level and storage level was surveyed and marked by CWI per- <br />sonnel, <br />
