The report includes location and site maps, photographs <br /> showing the current condition of the site, and a statement in lieu of a description of <br /> reclamation work completed indicating that no new surface disturbances were created <br /> subsequent to issuance of the NOI. The permitted NOI includes a total of 3.88 acres and <br /> has a current bond of$18,711.
