Cedar St. <br />Julesburg, CO 80737 <br /> <br />RE: Sedgwick County Road and Bridge Laster Pit, File No. M-2024-005 <br /> Receipt of 112c Construction Materials Reclamation Permit Application Package <br /> <br />Dear Kasey Campbell, <br /> <br />On February 26, 2024, the Division of Reclamation, Mining and Safety (Office) received <br />your 112c Construction Materials Reclamation Permit Application package for the Sedwick <br />County Road and Bridge Laster Pit, File No. M-2024-005. Preliminary review of the information <br />received determined that the following additional information must be received before the Division <br />can consider your application as being submitted and technical review can begin. <br /> <br />1. Provide inspection contact information on page 11 of the application. <br /> <br />2. Initial the “Responsibilities of the Permittee” List on pages 5 and 6 of the application. <br /> <br />3. Provide a fully executed certification page of the 112c application (page 8). <br /> <br />4. Provide proof of the Board of County Commissioners and the Local Conservation <br />District received the required notification per Rule 1.6.2(1)(a). The email <br />communications provided do not provide documentation of receipt. <br /> <br />5. Provide a signed Addendum No. 1 – proof of the required notice sign posted per Rule <br />1.6.2(1)(b). <br /> <br />6. Provide Exhibit N – Source of Legal Right to Enter. The submitted access agreement is <br />not notarized for the signature of the landowner. <br /> <br />Your application will not be considered submitted until all the information listed above is <br />received and found sufficient to begin our review. <br /> <br />You have sixty (60) days from the above date to submit all necessary documents that the Office <br />needs for an application to be considered filed. If, at the end of the sixty-day period, the application <br />has not been determined to be filed with the Office, the Office may deny the application and <br />terminate the application file.
