Colorado Inactive Mine Reclamation Program <br /> CLOSEOUT REPORT <br /> ALPS JUNCTION BOND FORFEITURE PROJECT <br /> Introduction: The Alps Junction Quarry is located in Lake County on private property approximately 3.6 miles east of <br /> Leadville and accessed from County Road 38.The site was permitted as a 110c quartz mining operation that encompasses <br /> 2.8 acres. On March 2, 2020, the bond was revoked and the financial warranty forfeited for failure to submit the required <br /> annual report, map and fee. <br /> PERMIT INFORMATION <br /> Permit Number Operator Name Mine Name <br /> M-1995-006 Leadville Research & Development Corp Alps Junction Quarry <br /> Type of Mine: 110c— Limited Impact Operation, Quartz Mine Commodity Mined: Quartz Riprap&Aggregate <br /> PRE-RECLAMATION SITE CONDITIONS: <br /> The Alps Junction site consisted of an open pit approximately one (1) acre in size with a highwall along the southeast side <br /> roughly 30 feet in height. Several small stockpiles of various sized material were present on the pit floor and approx. 25 <br /> concrete blocks and piers had been hauled to the site and stockpiled. Random trash and debris were scattered throughout <br /> the permitted area. Structures on site included an old grizzly screen with conveyor assembly, at-grade truck scale, 10'x8' <br /> wooden shed, and a 6'x8' concrete slab. <br /> BIDDING/CONSTRUCTION INFORMATION <br /> Type of Bid: Documented Quote - Bid in the Ocean Wave Project, DQ1-PKAA-2024*0040 <br /> The Alps Junction reclamation work was combined with the larger Ocean Wave Project to realize cost savings on <br /> mobilization and SHEAP tasks. The Ocean Wave Project consisted of five (5) safety closures for Newmont Mining at a <br /> total project cost of$128,092.75 ($44,710.00 Alps Jct + $83,382.75 Ocean Wave Safety Closures) <br /> PO Number Contractor Name &Address Construction Dates Bond Amount <br /> PKAA-2024*3915 $ 10,923.00 <br /> Epic Construction & Environmental, Inc. Backfilled <br /> material was graded to a 1.5:1 slope, roughened and seeded for revegetation. All borrow and disturbed areas were also <br /> roughened and reseeded. All trash and debris was removed from the site and properly disposed of. Large boulders were <br /> placed at the entrance to the scale to prevent use and damage.
