To complete the process of bid award,the following information must be received by me as soon as possible: <br /> 1. A certificate of insurance and Additional Insured Endorsement, in the amounts given in the Special Conditions of the <br /> project or Article 3, INSURANCE, of the General Conditions. We will use the certificates of insurance we have on file. <br /> Please review the attached to confirm coverage. <br /> 2. CONTRACTOR must provide documentation of current HAZWOPER training for all personnel that will be working on <br /> site to the PROJECT MANAGER at the time the Notice to Proceed is issued. Delivery drivers not working on site are <br /> exempt from this requirement. <br /> 3. Sales Tax Exemption: Per the Colorado Dept. of Revenue Publication, Sales 18 (08/94). The contractor must file an <br /> application for an exemption certificate on a form prescribed by the Department of Revenue. If approved, the <br /> department will issue an exemption certificate to the contractor. You will need the DRMS Tax Exemption number to <br /> apply for the certificate. The number is 09817835. <br /> When the above documents have been submitted and found to be in order, and the agreement fully executed, the Notice <br /> to Proceed will be given by the Project Manager.
