Construction <br /> Ltd., for the Loesch Pit, Permit M-1981-068. The following items are incomplete for the <br /> application to be processed: <br /> 1. Legal Right to Enter: No documentation from the Prospective Applicant was provided with <br /> the application providing the Division with proof of the legal right to enter the property. <br /> Please provide documentation in the form of a deed for the land containing the permitted <br /> area or a lease agreement. <br /> 2. An original Affidavit of Authority signed by Sean Mello and notarized. <br /> 3. An original Financial Warranty form signed by Sean Mello and notarized along with a bond <br /> payment for$30,468.00. 1 can supply a copy of this form via email once you confirm what <br /> method of payment you'll be utilizing (check, cd, corporate surety, letter of credit). <br /> Please return the required documentation to the Division before December 31, 2023. All <br /> completed permit documents are required to be submitted for consideration of your Request for <br /> Transfer of Mineral Permit and Succession of Operators Application. An incomplete application <br /> may be denied after the 60 day submittal request, if requested documents or information are <br /> not received.
