ORDER <br /> Based on the foregoing findings of fact and conclusions of law, the Board <br /> hereby APPROVES the Application with the following conditions: <br /> 1. The City of Lafayette shall install the proposed monitoring wells by June <br /> 30, 2023 and shall submit to the Division, within 30 days of completion of <br /> monitoring well and installation activities, the boring logs, well <br /> construction diagrams, and copies of the proper Department of Water <br /> Resources submittals. <br /> 2.
