S <br /> Colorado Inactive Mine Reclamation Program <br /> CLOSEOUT REPORT <br /> BOND FORFEITURE PROJECTS <br /> Introduction: The Donna Julia Project is located in Clear Creek County, west of Idaho Springs and adjacent to the <br /> historic Stanley Mine south of Interstate 70. The purpose of this project was to reclaim this hard rock mine, in <br /> accordance with the approved mining and reclamation plan. The Donna Julia Prospecting Notice of Intent (NOI) was <br /> revoked and the financial warranty was forfeited on October 14, 2011. <br /> Permit Information <br /> Permit Number Operator Name Mine Name <br /> P-1994-008 Kevin Bettale &Douglas Barrnet Donna Julia Mine <br /> Type of Mine-Metal Commodity Mined-Gold and silver <br /> PRE-RECLAMATION SITE CONDITIONS <br /> The Donna Julia Mine was an underground gold and silver mining operation on the Lincoln Claim. Mining at the Donna Julia <br /> Mine began in 1994. It appears,from the extent of the disturbance and the depth of the adit tunnel, that a significant amount <br /> of underground development was done during this period, and possibly prior to lawful permitting. The total disturbed area <br /> covers less than one (1) acre. The State of Colorado, Division of Reclamation, Mining and Safety revoked the Donna Julia <br /> Mine permit and forfeited the bond in 2011. At the time this project began a significant amount of mining-related debris still <br /> remained onsite along with the aging portal brow awning. The site was photographically inventoried for posterity, and to <br /> ensure evidence exists on file with DRMS as to the contents of the site. <br /> BIDDING/CONSTRUCTION INFORMATION <br /> The project consisted of removal of a pre-existing adit door and replacement with a grated adit door closure with bat access. <br /> This site was reclaimed using the forfeited bond money and State Severance Tax funds. <br /> Type of Bid Direct Bill - Donna Julia Project, was a Direct Bill <br /> PO Number-N/A Contractor Name&Address Construction Dates Bond Amount-$950.00 <br /> RMC Consultants <br /> PO Date-N/A 12295 W. 48th Ave Start: 8/13/2012 Project Cost-$4871.00 <br /> Wheat Ridge, CO 80033 End: 8/17/2012 <br /> Construction Work Description: During the course of the project John Miner, resident of <br /> Clear Creek County, assumed ownership of the Lincoln <br /> 1. Sample, analysis, and waste determination of one 55 Claim. All work was completed in accordance with the <br /> gallon drum approved reclamation plan, and with the new owner's <br /> 2.Sample analysis, and waste determination of one 5 interests in mind. Additional onsite work would have <br /> gallon bucket involved collection and disposal of all potentially <br /> 3.Sample and analysis of 3 potential asbestos samples hazardous mining-related debris and liquids. However, <br /> 4. Installation of one 9x9 adit closure with: the current landowner balked at the mention of this <br /> a. 7-8 foot tall by 2 foot wide door with bat ladder additional work due to concerns surrounding the <br /> b. Concrete footer with drain pipe (poured over perceived value of the site debris. As a result of this <br /> existing train rails) reluctance DRMS did not implement the proposed second <br /> 5.Removal of existing adit closure doors and set aside phase of construction that would have included debris <br /> 6.Repair of gaps between existing roof and bedrock removal.
