John Jay Mine <br /> Type of Mine— Underground Commodity Mined- Gold <br /> PRE-RECLAMATION SITE CONDITIONS <br /> The John Jay Mine is located on private property and USFS managed land 2.5 miles west of the town of <br /> Jamestown in Boulder County, Colorado. The site is adjacent to James Creek and access is from Gold Lake on <br /> County Road 102J. A prospecting Notice of Intent (NOI) was issued to Fatima Minerals in 1983. The prospecting <br /> NOI was revoked and the bond money forfeited in 2000. The initial bond of$2,000.00 was used to safeguard the <br /> shaft and no funding was available to reclaim the remainder of the site. The remaining disturbance at the John <br /> Jay Mine includes the 2000 reclaimed shaft, mine buildings, storage trailers, numerous demolished trailers, wood <br /> piles, old mining equipment, vehicles and mining related debris and trash. The landowner has requested the <br /> shaft head frame to remain on site for historical purposes. The landowner was notified to remove anything of <br /> value so the area could be reclaimed and various pieces of old mining equipment were removed. Prior to <br /> contracting the demolition, structures were tested for asbestos related material to determine how the material <br /> should be handled and disposed. Asbestos abatement was a component of the demolition project. <br /> BIDDING/CONSTRUCTION INFORMATION <br /> Type of Bid Documented Quote Purchase Order <br /> (circle one) Invitation for Bid <br /> PO Number: Contractor Name &Address: Construction Bond Amount- $0.00 <br /> PKA-11-641 Dates: Severance Tax- $41,672.60 <br /> Left Hand Excavating USFS Cost- $82,620.40 <br /> Start: 8/30/2010 <br /> PO Date 3756 Eureka Way End: 10/4/2010 Project Cost: $124,293.00 <br /> Frederick, CO 80027 <br /> Construction Work Description <br /> Construction began on August 30, 2010 with equipment and materials being mobilized to the project site and <br /> sediment control measures installed around the disturbed area. The asbestos survey was performed at the John <br /> Jay Mine on June 22, 2010 by ATC Associates Inc. Eighty-two (82) bulk samples were collected from various <br /> structures, trailers and debris piles at the John Jay Mine. The results from the survey indicate that eighteen (18) <br /> materials were identified as asbestos containing materials (ACM) at the Lower, Middle and Upper John Jay sites. <br /> Major Asbestos Control, Inc. was the subcontractor used to remove all asbestos material prior to demolition of <br /> the given structures. The asbestos containing materials were removed, stored in clean double bagged goose <br /> neck bags, transported and disposed of in accordance with all Federal, State and County rules and regulations. <br /> Once the asbestos containing materials were removed, structures and trailers were demolished and sorted into <br /> various piles for recycling, scrap metal and trash. The Contractor obtained a building demolition permit (also <br /> referred to as a Deconstruction Permit) from Boulder County. Construction material was sorted into the various <br /> piles at the mine site and hauled to the upper landing area for transport. Scrap material ultimately ended up at <br /> Rocky Mountain Recycling in Commerce City, the tires to Snowy River Enterprises in Longmont and the debris to <br /> Republic Services and the BFI Landfill off of Highway 93. <br /> A small subsidence feature was developing near the John Jay Shaft that was reclaimed in 2000. The Division <br /> instructed the contractor to excavate the area to determine the extent of the subsidence. The material was loose <br /> and wet. At a depth of eight feet (8') a small drift was observed. At a depth of 15 feet the hole was no longer <br /> apparent and seemed to be filled in. The subsidence was filled with adjacent large rocks and material excavated <br /> from the area. The trench was backfilled, compacted and revegetated. <br />
