A mining and reclamation permit was issued to Lord Byron
Gold Mining in 1988. The prospecting NOI has been revoked and the bond money was forfeited in 1988. The
reclamation on site using the forfeited bond money and severance tax money includes securing the adit/decline
with a grated adit closure allowing access, removal of old mining equipment and debris, and revegetation of all
disturbed areas.
BIDDING/CONSTRUCTION INFORMATION
Type of Bid Documented Quote Purchase Order
(circle one) Formal - Bid as the OE PKA 10000055
PO Number Contractor Name &Address Construction Bond Amount- $500.00
OE PKA 10000055 Dates
Berry Trucking and Excavating,
Inc. Work started on the
road leading up to the mine where trash and debris was removed. The structure and all other remaining debris in front of the
adit/decline was removed with a New Holland Backhoe. A forty foot(40')culvert with bat ladder was then installed into the
adit. Rock from the nearby waste pile was used for backfill around the culvert and topsoil from initial mine dig out was set
aside and then placed on top of waste rock to encourage revegetation. Soil around adit was then graded to blend into the
surrounding topography. Surface roughening also took place to trap sediment/water runoff as well as the construction of a
small berm at the toe of the slope for sediment control purposes. Two (2)tons of certified weed free mulch per acre and the
Western Pinion/Juniper seed mixture were applied to all disturbed areas. A total of 1 acre was reclaimed. The total cost for
this work was $10,795.00.
Change Order#1- No additional costs were associated with this change order. The project completion date was extended
with an additional 12 days given wet spring conditions and amount of time for culvert to arrive. Also, additional work was
required at the Lord Byron Mine that was not previously noted in the scope of work. Equipment rental hours (10 hrs at
$105.00/hr)was converted into specific tasks that required completion. The additional tasks included salvaging topsoil
around the Lord Byron adit and spreading it over the waste rock material, smoothing out the waste pile area and blending it
in with the surrounding area, the removal of wood fencing on the eastern side of the Lord Byron structure, and replacing the
fence post and cable along access road as to prevent access to the site.
The project was completed on June 25, 2010.
Remaining Reclamation Requirements and Estimated Cost:
There are no additional reclamation requirements for the Lord Byron Mine.
/7 SI
Project Manager -nior Project Manager
*41.2 i ) 29 Jciu-r—?3/o
Date Date
