Colorado Inactive Mine Reclamation Program <br /> CLOSEOUT REPORT <br /> ACTIVE MINE BOND FORFEITURE PROJECTS <br /> Permit Information <br /> Permit Number Operator Name Mine Name <br /> M-1980-109 Gold Ridge Mining Company Moose Mine <br /> Type of Mine— Underground Commodity Mined-Gold <br /> PRE-RECLAMATION SITE CONDITIONS <br /> The Moose Mine Bond Forfeiture site is located on private land located seven (7) miles northeast of the town of Idaho <br /> Springs in Gilpin County, Colorado. A mining and reclamation permit was issued to Gold Ridge Mining Company in 1980. <br /> The permit was revoked and the financial warranty forfeited by the State of Colorado in 2005. The Moose Mine consists of <br /> a hard rock mine shaft, old mining structure, and old mining equipment. The remaining reclamation on site using the forfeited <br /> bond money includes securing the shaft, removing old mining structures, debris, and trash, minor grading and reseeding. <br /> BIDDING/CONSTRUCTION INFORMATION <br /> Type of Bid Documented Quote Purchase Order <br /> (circle one) Formal— Bid as the PKA-10-1184 <br /> PO Number Contractor Name &Address Construction Bond Amount-$500.00 <br /> PKA-10-1184 Dates <br /> RM2, LLC Start: 7/12/2010 <br /> PO Date P.O. Box 266 End: 7/21/2010 Project Cost-$ 9444.00 <br /> N/A Loma, CO 81524 <br /> Construction Work Description <br /> Construction began on July 12, 2010 with equipment and materials being mobilized to the project site. Work started on the <br /> shaft where an iron cross bars were installed to make a grated closure with access. A concrete footer was placed on the <br /> shaft site to prevent additional piping. The mine hoist building was previously tested for asbestos material and found <br /> negative. The structure was completely torn down with little salvage, and hauled off by Green Waste and Recycling to a <br /> landfill. Old mining equipment such as steel fuel containers, air compressor, trailer, pipes, beams and miscellaneous trash <br /> were also hauled away by the contractor for salvage. The hoist frame and shed were left in place at the request of the <br /> landowner. All of the debris was cleared from the site, and all disturbed areas were scarified, mulched, and seeded. Two (2) <br /> tons of certified weed free mulch per acre was applied to all disturbed areas. The areas were seeded with the Ponderosa <br /> Pine/Spruce/Fir seed mixture. A total of 1.0 acres was reclaimed. The total cost for this work was$8,444.00. <br /> Change Order#1: <br /> Item#1- Bats were observed on site, therefore, a bat closure was included in the closure of the shaft. RM2 constructed a bat <br /> couplea to allow bats to enter and leave the mine workings. A portion of task#5 (equipment hours)was converted to cover <br /> the bat couples cost of$250.00. <br /> Item#2- Erosion repair work on the hillside was warranted. RM2 surface roughened the two roads on the hillside to provide <br /> a seed bed preparation. An area approximately 500 ft long by 12 ft wide was left in a roughened state and revegetated in <br /> accordance with the specifications. A portion of task#5 (equipment hours)was converted to cover the grading/erosion <br /> control work and revegetation cost of$750.00. <br /> The project was completed on July 21, 2010. <br /> Remaining Reclamation Requirements and Estimated Cost: <br /> There are no additional reclamation requirements for the Moose Mine Site. <br /> Project Manager Senior Pro Hager <br /> 2(2/ 1 2#5/ <br /> Date Date <br />
