Additional Bid Items (Task 12-Task 15): All additional bid items were utilized during construction including additional <br /> equipment rental hours, additional rip-rap and additional revegetation. These additional bid items are noted in the Change <br /> Order section below. <br /> Additive Alternates (Task 16-Task 20): Because the base bid was within the budget, all of the additive alternates were <br /> included in the project. This included eight(8)additional drop structures to be constructed in the upper channel (for a total <br /> of 15)and the grouted boulder rundown below Culvert C3. <br /> The initial base bid price was $428,509.90. With the addition of all of the additive alternates the total contract price was <br /> $507,457.82. <br /> Change Order#1: Allowed CCC to use the water located in Pond 4 as dust suppression during demolition of the Franklin <br /> Mill. Water from Pond 4 was contained in the working area and no water was discharged off site. There was no increase in <br /> cost for this change. The second item was additional surveying, leveling and staking of drop structures above Culvert Cl. <br /> The final surveying cost was $1,467.44. <br /> Change Order#2: Pond 4 was permitted as a water recirculation pond for the Mill, but ended up containing 7-9 feet of <br /> tailings material. Equipment hours were used to excavate the unforeseen tailings. Due to the nature of the tailings material <br /> in Pond 5, the grading plan was revised to keep the Pond 5 embankment in place and move the lower drainage channel 60 <br /> feet to the west. The tailing/overburden material from Pond 4 was mixed with 120 tons of portland cement to create a less- <br /> permeable cover and placed in the 0.6 acre footprint of Pond 5. Twenty-four inches (24")of clean fill material was placed <br /> over the cemented cover material. The backfilled area was then graded to a 2 percent slope towards the southwest. An <br /> earth lined ditch was constructed to route flows away from the tailings pond. The change order increased the contract <br /> amount by$22,424.56. <br /> Change Order#3: Equipment hours were used to further deepen and define the earth lined channel above USC-7, and <br /> additional cost was needed for the cemented cover soils. In addition, a rock lined spillway was installed on Pond 5 <br /> embankment in the event storm water entered and flowed across the backfilled pond area. The change order increased the <br /> contract amount by$1,277.57. <br /> Change Order#4: An illegal industrial well was found during reclamation at the Franklin Mine. Field measurements <br /> indicated the well was 300 feet in depth and dry. The pump on the well was pulled and scrapped. The well was backfilled <br /> with clean gravel, 2 inch minus rock, and inert material located on site. The top 5 feet was plugged with concrete. <br /> Abandonment of the well was in accordance with the Division of Water Resources, Water Well Construction Rules and <br /> Regulations. The cost to abandon the well was $532.56. <br /> The change orders noted above increased the final construction cost to $533,159.95. Work was completed on October 20, <br /> 2009, approximately two weeks ahead of schedule. <br /> Remaining Reclamation Requirements and Estimated Cost-The Franklin Mine site will be inspected annually until <br /> vegetation has established and the storm water permit can be terminated. <br /> S76.De 25-7(1---1P97 <br /> Project Manager ,'-'-nior Project Manager <br /> 112( 20)0 13- J 11..)
