Compliance With Mine Plan NA - Support Facilities Not On-site <br /> NA - Other NA - Special Categories Of Mining <br /> NA - Processing Waste N - Topsoil <br /> COMMENTS <br /> This was a complete inspection of the Williams Fork Mine; permit C1981044, operated by <br /> Moffat County Mining LLC. The inspection was conducted and the report and photos prepared <br /> by me, Robin Reilley of the DRMS. The temperature was approximately 20 degrees F. There <br /> were gusty winds under clear skies. The site was inaccessible due to snow <br /> PERMITTING ACTIONS in PROGRESS: <br /> MAINTENANCE ITEMS <br /> MAINTENANCE ITEMS ADDRESSED <br /> INSPECTION <br /> GENERAL MINE PLAN COMPLIANCE: <br /> Reclamation is mostly completed as permitted. Facilities have been removed throughout the site. <br /> Reclamation, backfilling, grading and positive drainage at the 5A Portal are nearly completed <br /> and is anticipated to be finished in Spring of 2023. Topsoil from nearby topsoil piles will be <br /> hauled and placed. The 5A portal area was disturbed after August 1977 and prior to April 1978. <br /> SIGNS and MARKERS Rule 4.02 <br /> o Permit identification signs were posted at the various entrances from Highway 13 and <br /> displayed all required information: mine name, operator name, address, phone number, <br /> DRMS permit number and contact information. <br /> ROADS Rule 4.03 <br /> Construction 4.03.1(3)14.03.2(3);Drainage 4.031(4)14.03.2(4); Surfacing and Maintenance <br /> 4.03.1(S) and(6)14.03.2(5) and(6);Reclamation 4.03.1(7)14.03.2(7) <br /> o The haul road at the entrance off State Highway 13 snow covered and rendered the site <br /> inaccessible. <br /> Number of Partial Inspection this Fiscal Year: 4 <br /> Number of Complete Inspections this Fiscal Year: 3 <br /> Page 2of4 <br />
