York, P.E. <br /> J&T Consulting, Inc. <br /> 305 Denver Avenue, Suite D <br /> Fort Lupton, CO 80621 <br /> Subject: Groundwater Monitoring Well Installation <br /> Mining Property Highway 263 <br /> Greeley, Colorado <br /> Project No. 22.3050 <br /> Dear Mr. York: <br /> CMT Technical Services - Colorado (CMTTS) performed drilling and groundwater monitoring well <br /> installation at the site of the proposed aggregate mine on Highway 263 in Greeley, Colorado. Figure <br /> 1 shows the location of the project site. <br /> Five borings were drilled on September 8, 9, and 12, 2022 using a truck mounted CME-55 drill rig <br /> equipped with 3-3/4 inch inside diameter hollow stem auger. A representative from CMTTS was onsite <br /> to log the borings and observe monitoring well installation. Figure 2 shows the project site with <br /> locations of the boring/monitoring wells. <br /> Subsurface conditions varied, but consisted of gravelly sands to sandy gravels, with localized layers <br /> of sandy clay. Claystone to sandstone bedrock was encountered in all five borings at depths ranging <br /> from 40 feet to 112 feet below ground surface. Groundwater was encountered in the five borings at <br /> depths ranging from 15 to 40 feet below ground surface. A PVC pipe monitoring well was constructed <br /> in each boring. The monitoring wells were constructed with 2 inch diameter PVC factory slotted and <br /> threaded pipe. Natural/native sands and gravels, 10/20 silica sand and bentonite chips were used at <br /> appropriate intervals as backfill material. The monitoring well riser pipe was completed at a height <br /> of approximately 2.5 to 3 feet above existing ground surface and protected with a steel stickup type <br /> monument embedded in a 2 by 2 foot concrete pad. A graphic log of the subsurface conditions <br /> encountered, locations of sampling, monitoring well construction details, and further explanation of <br /> the exploration are presented in the boring logs in Appendix A. <br /> Prior to its site visit, CMTTS filed the appropriate"Notice of Intent"form (GWS-51)with the Colorado <br /> Department of Water Resources (DWR). After completion of site services, CMTTS filed the <br /> appropriate "Well Construction Report" forms (GWS-31) with the DWR. The DWR acknowledged <br /> receipt of the GWS-31 forms. CMTTS also filed the well permit document(GWS-46) and paid the well <br /> permitting fees.
