Amendment 02 Adequacy Review Round 2 Questions and Responses DRMS Comment Text(from DRMS letter AR Round 2) OSMI Responses Comment# While a regional map(Map B-1)was included due to the scale,all roads and other access to the permit area was A revised Map B-1 with the additional features requested was developed.Please see updated Exhibit B in illegible.Either provide an additional smaller scale map depicting the access roads utilized to enter each of the Attachment 1. 1 sites associated with this permit or highlight on the current map,all access roads utilized to enter all areas of the affected lands pursuant to Rule 6.4.2.This should also include the road used to access the Yellow Rose Vent Shaft Area. The requirements of Rule 6.4.3(e)are not sufficiently addressed on maps C-1A or C-1B presented in Exhibit C.Vegetation information has been added to relevant maps in Exhibit C.Please see updated Exhibit C Maps in Although sufficient maps were provided in Exhibit J,Vegetation Information,they are required under Exhibit C Attachment 1. 2 pursuant to Rule 6.4.3(e).Please duplicate the vegetation maps in Exhibit C.Additional vegetation information may be presented on a map in Exhibit 1 in addition to Exhibit C,if desired. 3 On Map C-1B roads are a depicted feature.However the name of the road is not clearly defined as required by Map C-1B has been updated with a label to the road as requested.See Updated Exhibit C in Attachment 1. Rule 6.4.3(b)please revise Map C-1B.g Maps C-4(Groundwater Flow)and C-5(Regional Geology)should be included within Exhibit G as they are used to Map C-4 has been revised to be G-3 and Map C-5 has been updated to be new Map G-4.Section 2 of Exhibit G has demonstrate groundwater flow regimes in the area.Please remove these maps from Exhibit C and insert them in been updated to reference these new maps. Additionally please locate the Governors basin and Yellow Rose Permit Areas on Map C-5(Regional Geology). On Map C-3 please clarify if the'baseline conditions'depicted in orange are the current site conditions or prior to The"baseline conditions"depicted in orange are current site conditions of the Tailings Storage Facilities.These 5 construction of the Tailings Storage Facility(TSF).If not please include another cross-section layer that depicts the facilities have been labeled to more clearly reflect their status.Please see updated Map F-3 in Exhibit F. current site conditions. In the Mining Plan(Exhibit D)on page D-17 section 4.2,it stated that the avalanche paths are shown on map C-1A.A new map,Map C-6 has been developed that shows the avalanche slide paths located in route to the mine and in Map C-1A does not depict avalanche flow path information.Though this information is not required under Rules the vicinity of the Revenue TSF.Section 4.2 of Exhibit U has been updated to reference the new Map C-6.See 6 6.4.3 or 6.4.4,given the nature of the site and its high risk for avalanches,it is good information to include.Please Attachment 1, updated Exhibit D and Exhibit C for details. include a separate map with the local avalanche flow paths and update references(s)as necessary. 7 On Map C-1A please label Sediment Pond#1,located on the east side of the Atlas TSF. Sediment Pond#1,located on the east side of the Atlas TSF has been labeled on Map C-1A.See Attachment 1 for Updated Exhibits. Map C-1A is a map of the current/baseline conditions.Several features are proposed in this amendment such as Map C-7 was created to show proposed features that have not yet been constructed.See Map C-7 in Updated the Tailings Thickener Superstructure,proposed groundwater monitoring wells and Sediment Pond#2 but are not Exhibit C in Attachment 1 for details. 8 included on Map C-1A because they have not yet been constructed.Please provide an additional map that shows all proposed features as this will be used as the Mine Plan Map moving forward. Sections 4.2.1 reflects a change in total height of the Revenue TSF that is inconsistent with the table responses,The original build outs of the Atlas and Revenue TSFs that were approved under the 2015 Tailings and Waste associated maps and approved 2015 Tailings and Waste rock Management Plan (TWMP). Please revise the Management Plan remain the current designs for these facilities.Exhibit D has been updated to refer back to the 9 narrative to reflect the previously approved height,slope and benching requirements of the 2015 TWMP.Also,2015 TWRMP. the table responses to questions number 123 and 124 of the PAR provide additional information that clarifies the TSF design specifications.Please include that information in the narrative of Exhibit D. The additional information provided in response to question 123 and 124 from the PAR table has been incorporated into Section 4.2.1 of Exhibit D.Please see updated Exhibit D in Attachment 1. The'typical'raise info provided under section 3.3 does not coincide with what was built at the Governors Basin The surface features at the borehole are as follows. Raise Bore.Describe the surface structure(s)and equipment currently present or to be installed at the Governors-Borehole=8'diameter Basin Raise Bore.Explicitly state the existing shaft diameter,concrete pad(s)size,hoist housing/shed construction-Headframe=pad size is 14'x 19'(minus the 8'diameter borehole)plus two 2'x 2'footers type(or other buildings)and dimensions that will eventually need reclaimed. Hoist building has no concrete foundation-it's comprised of two 20'x 8'Connex containers 10 Concrete pad for former winch=10'x 10' Refuge Chamber=two,40'x 8'Buried Connex Containers Refuge Chamber access=50 feet of 6'diameter corrugated steel.
