Describe Proposed or Completed Operation:Clearly state all pertinent details,including estimated starting date of any proposed work and approximate duration thereof.If the proposal is to deepen directionally or recomplete horizontally,give subsurface locations and measured and true vertical depths of all pertinent markers and zones.Attach the Bond under which the work will be performed or provide the Bond No.on file with BLM/BIA.Required subsequent reports must be filed within 30 days following completion of the involved operations.If the operation results in a multiple completion or recompletion in a new interval,a Form 31604 must be filed once testing has been completed.Final Abandonment Notices must be filed only after all requirements,including reclamation,have been completed and the operator has determined that the site is ready for final inspection.) All depths are referenced to GL.The DS-2 well reached the end of its useful life and has been plugged and abandoned.The transducer and associated cable was pulled out of the well.A 3.75-inch tri-cone bit was advanced down the well to air lift and clean the hole with foam water injection to clean the hole to a depth of 1,854 feet.The casing was then cleaned with a 4.5-inch casing scraper to 1,825 feet.A cement bond log(CBL)was run from a depth of 1,829 feet to surface to identify perforation zones.The 4.5-inch casing was perforated at 1,312, 1,140, 1,018,915,535,250,and 50 feet below ground level with 4 shots per foot at 90 degree phasing.All cement plugs were placed using the balance method.Plug 1 cement consisted of 15.6 bbls(68.3 sacks)of 14.3 lb/gal Portland Type 1/11 cement and was pumped in three lifts:350 gallons of cement at a depth of 1,840 feet,200 gallons at 1,640 feet,and 107 gallons at 1,440 feet.Plug 1 was allowed to set 96 hours,after which the top of cement was tagged at 1,017 feet.Plug 2 cement consisted of 16.8 bbls(73.5 sks)of 14.3 lb/gal Portland Type 1/II cement and was pumped in three lifts,the first lift of 300 gallons from 1,015 feet,200 gallons from 715 feet, and 150 gallons at 415 feet to surface.After the tubing was removed from the well the 4.5-inch casing was topped off to surface with 55 gallons of cement. The well was dug out at surface and the casing was cut off 3 feet below GL.An additional 0.3 bbl(1.3 sks)of Portland Type I/11 cement was poured down the 4.5-inch casing annulus to fill it from 15 feet to surface.A steel cap with the well name was welded onto the 7.0-inch conductor casing and the well was buried. The calculated volume of the 25 feet of 3.875-inch open hole and 1,829 feet of 4.5-inch OD cased hole was 29.53 bbls(165.8 cu ft).A total of 32.7 bbls(143.1 sks, 183.6 cu ft)of cement, 10.7%excess,was used to plug and abandon the DS-2 well to surface.The DS-2 plug and abandonment was completed on 9/21/22.
