PERMIT#:M2020-002 <br /> INSPECTOR'S INITIALS:RAR <br /> INSPECTION DATE: 4 August 2022 <br /> OBSERVATIONS <br /> The inspection comprised a regular monitoring inspection. The Permittee (Freemont Paving and Ready Mix, <br /> Inc.),was represented by Jodi Shreiber throughout the inspection. The Horse Creek Pit is accessed from <br /> Highway 71 and is located approximately 35 miles south of Limon. This is a I I I permit for sand and <br /> gravel for construction for a government agency. <br /> Availability of Records: The site is permitted for 30 acres,with an approved post-mining land use of <br /> rangeland. There are no open infractions. No previous inspections are on record. <br /> Financial Warranty: This pit is permitted by Freemont Paving and Ready Mix,Inc, and DRMS holds a <br /> bond of$75,000.00 as required by the Division. <br /> Fish and Wildlife: No impact to fish or wildlife was observed. <br /> Hydrologic Balance: No standing water was observed in the pit and no exposed groundwater was <br /> observed. Any runoff or snow melt drains into the pit and infiltrates into the sandy soils. <br /> Compliance with Mine Plan: The operation appeared to be in compliance with the approved mine plan. <br /> No activity was observed at the time of inspection and all processing facilities had been dismantled. Pit <br /> slopes are currently nearly vertical(Photo 2), and will be reclaimed to a slope of 3:1 or more gentle. <br /> Off-site Damage: The operation appeared to be confined to the permit boundary. <br /> Processing Waste: No processing waste was observed at the time of inspection. <br /> Roads: There is no defined road. <br /> Reclamation Success: No reclamation had been initiated at this site at the time of the inspection. A <br /> conversion to a reclamation permit is anticipated for later in 2022. <br /> Revegetation: No revegetation had been initiated at this site at the time of the inspection. The site has <br /> sparse vegetation mostly made up of minor annual weeds(Photo 4). Mitigating the weeds prior to <br /> seeding potentially this fall,was discussed. <br /> Sediment Control: No active erosion was observed at the time of inspection. There was minor Tilling <br /> noted on the overburden stockpiles(Photo 3). <br /> Signs and Markers: The permit sign was missing from the posted plaque at the entrance(Photo 1). <br /> This was noted and a replacement sign will be posted. The operator will send DRMS a photo of the new <br /> sign posted at the entrance. A fence encloses the permit boundary. <br /> Permit Stipulations: There are no permit stipulations. <br /> Storm Water MGT Plan: No oil or fuel spills observed. Stormwater drains to the pit and infiltrates. <br /> Page 2 of 5 <br />
