PERMIT#:M-1982-121 <br /> INSPECTOR'S INITIALS: CCW <br /> INSPECTION DATE:May 23,2022 <br /> OBSERVATIONS <br /> This report documents the observations made on May 23, 2022 at the Mid-Continent Mine, permit no. M-1982- <br /> 121. The Inspection was conducted by Clayton Wein and Amy Yeldell of the Division. Robert Wagner <br /> represented the operator RMR Aggregates, Inc. during the inspection. The Division conducted the inspection in <br /> response to a complaint about possible fugitive dust from the mine site. The weather was clear with a <br /> temperature of 70 degrees F. The ground conditions were dry. <br /> Air Resource Protection: <br /> The Division was notified that complaints made to MSHA and the Colorado Department of Public Health and <br /> Safety regarding excessive dust originating from the mill. The Division visited the mill during the inspection. <br /> The mill was observed to be inactive at the time. The operator identified the filters being disconnected from <br /> their housings as the source of the dust. The operator had already reconnected one of the filters and was <br /> preparing to reconnect the others (Photo 1). The Division observed no excessive dust from the site during the <br /> inspection. The operator was employing the required dust suppression methods. <br /> Hydrologic Balance: <br /> During the inspection the sediment pond, located to the south of the mill, was holding water (Photo 2). There <br /> was no discharge from the spillway. The inlet and spillway were observed to be stable with no erosional <br /> concerns. The embankment for the pond was stable and there were no indications of erosion. Diversion <br /> ditches throughout the mine site were clear of debris and capable of functioning as designed. There were no <br /> sections of the ditches observed to have the potential for water to escape the channels. Culverts at the site <br /> were generally in good condition and none were observed to be blocked by debris. <br /> Roads: <br /> Access roads throughout the mine site were stable with no erosional features observed. The roads were well <br /> maintained and dust suppression methods were being utilized. <br /> Support Facilities On-site: <br /> The mill was inactive during the inspection. The mill was well kept and organized. All hydrocarbons that were <br /> stored in the building were kept on spill retention pallets for secondary containment (Photo 3). There were no <br /> spills observed in the building. <br /> Fuel was stored in diesel containment tanks (Photo 4). The tanks were in good condition and no spills were <br /> seen. <br /> The crusher was located on the processing/highwall bench. The crusher was inactive during the inspection. <br /> The bench was well organized and there were no off site impacts noted. The bench was stable and there were <br /> no erosional features. <br /> Signs and Markers: <br /> The Mine Identification sign was not posted at the entrance to the site. Discussions with the operator <br /> revealed the sign was damaged and was in the process of ordering a new one. Please replace the Mine <br /> identification sign. Once the sign is replaced, please provide the division with photographic evidence that the <br /> sign has been replaced at the entrance to the site. <br /> Page 3 of 7 <br />
