Mahogany Energy Resources, L.L.C. <br /> 285 Bch Street <br /> Meeker, CO, 81641 <br /> 970-878-9995 <br /> May 31, 2022 <br /> Colorado Division of Reclamation Mining&Safety, <br /> RE:Annual Report for File No. 2021-002; Shale Core 2020-01, Notice of Intent to Conduct Prospecting <br /> Operations for Hard Rock/Metal Mines. <br /> The following activities have taken place after the 5-business day hold period following the NO[ <br /> acceptance of Shale Core 2020-01 (File No. 2021-002). <br /> • June 15, 2021 to July 1, 2021: Pad site preparation. The top soil for a 100' x 150' area was <br /> stockpiled at the perimeter of the pad site. The site was then leveled for drilling operation. A <br /> small access road was improved per the plan to the pad site. <br /> • July 5, 2021 to July 8, 2021: Casing was set for two core holes on the pad. An 8%" bit was used <br /> to drill 63 feet for the two holes on the pad. 6 5/8" casing was set to a depth of 63 feet for each <br /> hole and cemented in place. <br /> • July 9, 2021 to Sept 3, 2021:The first core hole was drilled with a PQ sized bit to a total depth of <br /> 1506.5 feet under surface. <br /> • Sept 4, 2021 to Sept 30, 2021:The second core hole was drilled with a PQ sized bit to a total <br /> depth of 1514.5 feet under surface. <br /> • November 2021:The permit approved seed mixture was added to the topsoil around the pad as <br /> it would not be reclaimed within 6 months. <br /> • November 2021: Geophysical downhole logging was performed on the top 1140 feet of the <br /> second core hole. <br /> • April 26, 2022 to May 13, 2022:The first core hole was filled with bentonite grout from the <br /> bottom to depth 7 feet under surface. The hole was then cemented to a depth 2 feet under <br /> surface. The remaining casing was cut off and removed before the hole was backfilled with <br /> native soil. The second core hole was filled with bentonite grout to a depth of 800 feet under <br /> surface. A piezometer was installed in the second core hole at a depth of 781 feet under <br /> surface. This was tremie filled with a cement/bentonite grout mixture per the manufacturer's <br /> recommendations. The top 5 feet of the hole was filled with concrete to seal the hole. <br /> • The site is now ready for reclamation.
