You must submit a financial <br /> warranty in this amount for us to issue a permit. On January 26,2022,you included a financial warranty in the amount <br /> of$25,000.00 in the form of a Corporate Surety. The Division was not able to accept this financial warranty. Here are <br /> your options to secure a financial warranty: <br /> 1) The originally submitted Financial Warranty in the amount of$25k wasn't signed nor notarized on behalf <br /> of Castle Rock Construction Company of Colorado, LLC (Operator). Piecemeal documents are <br /> acceptable;the Operator can sign and have notarized new pages 5 and 6 then submit an original Liberty <br /> Mutual rider increasing the bond to$48,300.00. <br /> 2) The original bond paperwork that is missing the Operator's signature can be disregarded. Instead,you may <br /> submit a new, original Financial Warranty Corporate Surety for the revised $48,300.00. This document <br /> needs to be signed and notarized on behalf of both Liberty Mutual and the Operator. <br /> If you choose to select a different type of financial warranty,please select a type of financial warranty from Rule 4.3. <br /> Then contact us so that we can provide you with the appropriate warranty form. <br /> PLEASE NOTE THAT MINING OPERATIONS MAY NOT COMMENCE UNTIL A PERMIT HAS BEEN <br /> ISSUED BY THE DIVISION AFTER RECEIPT OF YOUR FINANCIAL WARRANTY.
