COLORADO <br /> Division of Reclamation, <br /> Mining and Safety <br /> Department of Natural Resources <br /> 1313 Sherman Street, Room 215 <br /> Denver, CO 80203 <br /> April 24, 2017 <br /> Iron Woman Construction & Environmental Services, LLC <br /> 5680 Emerson Street <br /> Denver, CO 80216 <br /> Re: Notice of Award, Hard Scrabble/S Bar C Bond Forfeiture Project, Bid Number PKA-17-779 <br /> Dear Sir or Madam: <br /> This letter is to inform you that your company is the apparent low bidder on Project PKA-17-779 the Hard <br /> Scrabble/S Bar C Bond Forfeiture Project. Q To complete the process of bid award, the following <br /> information must be received by me in the offices of the Division of Reclamation, Mining and Safety on or <br /> before the due date: <br /> 1) Four executed copies of the enclosed agreement. <br /> 2) A certificate of insurance and Additional Insured Endorsement, in the amounts given in Article <br /> 3, INSURANCE, of the General Conditions. The certificate of insurance must show the State <br /> of Colorado as an additional insured. Contractor and all subcontractors shall provide <br /> certificates showing insurance coverage required hereunder to the State within seven <br /> business days of the Effective Date of this Contract. No later than 15 days prior to the <br /> expiration date of any such coverage, Contractor and each subcontractors shall deliver to the <br /> State or Contractor certificates of insurance evidencing renewals thereof. In addition, upon <br /> request by the State at any other time during the term of this Contract or any sub-contract, <br /> Contractor and each subcontractors shall, within 10 days of such request, supply to the State <br /> evidence satisfactory to the State of compliance with the provisions of this §13. <br /> 3) A performance bond and a labor and materials payment bond in the amounts of 100 percent <br /> and 100 percent respectively of the bid amount ($146,759.54). The bonds must be submitted <br /> using the State forms. NOTE: Please ask your Insurance Company to leave the first page of <br /> the bonds undated. Please make sure the second page and also the Power of Attorney is <br /> dated. Copies are enclosed. <br /> 4) A project schedule, as specified in Article 5 PROJECT SCHEDULE of the General Conditions. <br /> 5) For contractors new to the state, or contractors who have not worked for the state recently, a <br /> W-9 form is attached. <br /> 6) As a payment option, a form for the contractor to receive an electronic payment is attached. <br /> 7) Sales Tax Exemption: Per the Colorado Dept. of Revenue Publication, Sales 18 (08/94), the <br /> contractor must file an application for an exemption certificate on a form prescribed by the <br /> Co. <br /> 1313 Sherman Street, Room 215, Denver, CO 80203 P 303.866.3567 F 303.832.8106 http://mining.state.co.us <br /> John W.
