DBA Asphalt Paving Co. <br /> 14802 W.44th Ave. <br /> Golden,CO 80403 <br /> Re: Ralston Quarry, Permit M1974-086, Succession of Operators Provisional Approval, <br /> Revision SOl <br /> Mr. Opperman: <br /> On February 8,2022,the Division of Reclamation,Mining and Safety provisionally approved the <br /> succession of operators from Asphalt Paving Company to Oldcastle SW Group, Inc.DBA Asphalt <br /> Paving Co. for this operation. When this revision is finalized,Oldcastle SW Group, Inc.DBA Asphalt <br /> Paving Co.will be the permitted operator of the Ralston Quarry,and as such,will be responsible for all <br /> provisions in M-1974-086,as well as those specified in the Rules and Regulations. Asphalt Paving <br /> Company will be relieved of all responsibilities concerning this operation. <br /> In order for this revision to be finalized,the successor operator will need to provide one of the following <br /> to the Division for approval: a name change rider for the existing bond or,a replacement bond in the <br /> same amount($3,680,700.00)with supporting documents. This material will need to be submitted to <br /> the Division within 30 days of the date of this letter via certified mail,or another trackable delivery <br /> service.You may also arrange to drop off the required documents in person at the Division office. <br /> Questions regarding surety submittal should be directed to Financial Assurance Specialist, Sara <br /> Stevenson-Benn. <br /> The transfer of this permit does not result in the transfer of any other permits or licenses with <br /> this Division, the State, or Federal agency which might be associated with this operation. <br /> If you have any questions,please contact me. <br /> Sincerely, <br /> ��V- javl— <br /> Eric C.
