Identify the primary, and if applicable, secondary and incidental commodities to be mined/extracted by <br /> the proposed operation. <br /> 12. Drainage Control: Describe the measures used to divert upland drainage away from the site both during <br /> and after operation.This must include design details demonstrating the capacity of ditches and <br /> impoundment structures to contain operating solutions and the volume of water generated by a one <br /> hundred(100)year 24-hour rainfall event. <br /> 6.3.4 Exhibit D Reclamation Plan <br /> 13. Specify at what point in the mining plan and to what depth(s)overburden will be replaced in relation to <br /> ongoing extraction. <br /> Sediment control and protection of hydrologic balance were not explicitly addressed in the Mine Plan. <br /> Reclamation of the topsoil pile was also not explicitly addressed. <br /> 14. Specify the reclamation treatment of any waste rock dumps,ditches,sediment control facilities,specified <br /> in your mine plan but not previously addressed in the Reclamation Plan narrative.These features must be <br /> shown on Exhibit E-Map.This should describe the measures taken to minimize disturbance to the <br /> hydrologic balance,prevent off-site damage,and provide for a stable configuration consistent with the <br /> proposed future land use. <br /> 15. A reclamation cost estimate was not included in the reclamation plan. Please provide the costs associated <br /> with each reclamation task. <br /> 6.3.5 Exhibit E Maps <br /> 16. Outline and label all major surface features to be used in connection with the proposed operation <br /> specifically pit boundary, overburden and product stockpiles on the Mine Plan Map. <br /> 17. State the average thickness of replaced overburden on the Reclamation Plan Map. <br /> 18. A berm is noted on the Mine Plan Map. Please provide additional detail as to this berm, is it for <br /> the Topsoil Stockpile or for another feature. <br /> 6.3.9 Exhibit I—Proof of Filing with County Clerk <br /> 19. A copy of the revised application needs to be placed on file with the local County Clerk and <br /> Recorder. Please provide an affidavit or receipt indicated the date the revised application was <br /> placed with the local Clerk and Recorder for public review. <br /> 6.3.10 Exhibit J Proof of Mailing Notices to the Board of County Commissioners and <br /> Conservation District <br /> 20. Please provide proof that a revised notice of the correct permit application was sent to the Board <br /> of County Commissioners and the Board of Supervisors of the local Conservation district. <br /> 6.3.12 Exhibit L Structure Agreements <br /> The Applicant shall provide information sufficient to demonstrate that the stability of any structures <br /> located within two hundred (200)feet of the operation or affected land will not be adversely affected. <br /> 21. Structure agreements for the four structures identified were not provided. <br /> The application will not be considered submitted until the information listed above is received and <br /> found sufficient to begin our review. A decision date will be established 15 days from the date of <br /> receipt of all of the requested information. <br />
