PERMIT 4: M-1978-314 <br /> INSPECTOR'S INITIALS: TNL <br /> INSPECTION DATE:December 2,2021 <br /> OBSERVATIONS <br /> This was an inspection of the Hewes Gravel Pit conducted by Tabetha Lynch of the Division of Reclamation, <br /> Mining and Safety. Weather conditions were clear and dry. It is located approximately 5 miles west of <br /> Toponas, CO and is accessed from Routt County Rd 3. No issues were noted as a result of this inspection. The <br /> Division holds adequate bond to reclaim this site. The current bond held is $86090.00. No additional bond is <br /> required as a result of this inspection. <br /> Gen. Compliance With Mine Plan: <br /> This is a 112 permit that consists of 341 acres with a max allowable disturbance of 15 acres at one time. <br /> Between 8 and 9 acres appeared to be disturbed at the time of inspection. The annual report stated 8.3 acres <br /> of disturbance. Mining is still within the original 110c permit boundary. Annual report, map, and fees have <br /> been received on time. <br /> The mine ID sign was present and shown in the photos below. Affected area markers were present. A <br /> permanent load scale and scale house are at the entrance (photo below). The site is an active site but was <br /> not operating at the time of inspection. A portable crusher was on site. No washing has been reported to <br /> occur on site. No fuel or hazardous materials are being stored onsite. The pit is at an estimated depth of 35- <br /> 40ft. Mining is moving north. No utilities run through the permit. Slope appeared stable at the time of <br /> inspection and vary from approximately 1:1 to 1:3. The pit has approximately 2,300 Ift of highwall. The bond <br /> calculation estimates 3,000 Ift of highwall as the worst case for up to 15 acres of disturbance. A few stock <br /> piles of product were present as shown in photos below. Topsoil is stored along the permit boundary and <br /> appeared stable. Evidence of weeds were present, and primarily appear to be Mullein. The operator stated <br /> that weeds are sprayed several times a year. The weed management plan should continue to be followed. <br /> The site has 5 groundwater monitoring wells as a result of revising the permit to a 112 permit. These wells are <br /> not currently monitored. Several raptor nests were observed in the permit area, but outside the affected <br /> area. <br /> No problems were noted during this inspection. <br /> Page 2 of 4 <br />
