The following contact documents have <br /> been provided and used to submit the Purchase Order <br /> 1) A certificate of insurance and Additional Insured Endorsement, in the amounts given in Article <br /> 3, INSURANCE, of the General Conditions. The certificate of insurance must show the State <br /> of Colorado as an additional insured. Contractor and all subcontractors shall provide <br /> certificates showing insurance coverage required hereunder to the State within seven <br /> business days of the Effective Date of this Contract. No later than 15 days prior to the <br /> expiration date of any such coverage, Contractor and each subcontractors shall deliver to the <br /> State or Contractor certificates of insurance evidencing renewals thereof. In addition, upon <br /> request by the State at any other time during the term of this Contract or any sub-contract, <br /> Contractor and each subcontractors shall, within 10 days of such request, supply to the State <br /> evidence satisfactory to the State of compliance with the provisions of this §13. <br /> 2) A project schedule, as specified in Article 5 PROJECT SCHEDULE of the General Conditions. <br /> 3) For contractors new to the state, or contractors who have not worked for the state recently, a <br /> W-9 form is attached. <br /> 4) All Vendors are now required to complete & return an EFT form unless they currently have a <br /> valid form on file. <br /> When the above documents have been submitted and found to be in order, and the contract agreement <br /> fully-executed, the Notice to Proceed will be given.
