B.3.5. Chain of Custody Procedure <br /> The sampling technician shall complete the chain of custody record during or after sample <br /> collection. <br /> • Use permanent ink or electronic entry to complete the form. <br /> • The initiator of the form is responsible for legibility of all entries other than signatures. <br /> • When the samples are physically transferred from one person to another, the relinquisher <br /> and receiver shall sign the appropriate block, with the date and time of sample transfer. <br /> The relinquishers, by signing, verify that the samples have been within their custody. <br /> • It is each signatory's responsibility to sign legibly. <br /> • The relinquisher retains a copy of the form. <br /> • Non-contractor employees are not required to sign the form (e.g., employees of shipping <br /> companies). <br /> • The following is the minimum information required on the form to ensure sample <br /> identification: <br /> • Sample ID (well name) <br /> • Date and time of sample collection <br /> • Number and types of sample bottles <br /> • All sample containers will use custody seals (Figure 5) or tags to seal the individual sample <br /> containers or the inner or outer shipping carton. <br /> • When seals are applied to the sample container, they must not obscure the information on <br /> the sample label. <br /> • Securely wrap or fasten shipping containers prior to application of the custody seals. <br /> • Enter the date the samples are sealed and sign the custody seals or tags as shown below. <br /> Clear plastic tape shall be applied over the seals for protection. <br /> Laboratories, Inc. <br /> Signature <br /> CUSTODY SEAL <br /> No. Date <br /> Figure 5.Example of Custody Seal. <br /> • An original chain of custody record shall accompany the samples until they are received by <br /> the laboratory. <br /> • Unless otherwise specified by the project, the chain of custody record shall be maintained <br /> as part of the project records. <br /> B.3.6.
