The site is in final reclamation,and not active
the day of the inspection. The weather was clear and dry. Tony Tennyson of Elk Ridge Mining and Reclamation
Accompanied the inspection and represented the Operator. Five Photos accompany this report to illustrate the
observed site conditions.
HYDROLOGIC BALANCE -Rule 4.05
Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10;
Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18:
During the inspection,aeration and mixing operations were active at Pond 13. The aeration and mixing
structure can be seen in Photo One. No problems related to this inlet structure were observed during the
inspection.
OTHER(SPECIFY):
The main focus of this inspection was to observe the proposed disturbance related to the recently
submitted Minor Revision(MR-121). The revision addresses the disturbance related to the installation of three
baffle curtains in Pond 13,to slow water movement and allow for more residence time for the water in the pond.
To install the baffle curtains,three sets of two Mooring Posts will be installed along the North and South sides of
the pond. The foundations for the Mooring Posts will be 10 inch diameter,four feet deep and set in concrete. The
post locations along the North bank have been delineated and marked with survey lathes, seen in Photo Two.
These posts are within the existing footprint of the pond,high on the North Banks. The locations of the posts on
the South banks were not delineated but the approximate locations can be seen in Photos Three and Four. To
safely install the posts,a small disturbance will be created to allow for equipment access approximately 6 feet
above the water line. This area will be graded to allow for safe access and installation of the posts. The route of
ingress and egress can be seen in Photo Five.
Number of Partial Inspection this Fiscal Year: 4
Number of Complete Inspections this Fiscal Year: 2
Page 2 of 6
