[Homeowner] <br /> [Homeowner address] <br /> [Homeowner address 21 <br /> SENT VIA UNITED STATES POSTAL SERVICE-CERTIFIED MAIL <br /> RE: Young Ranch Resource Quarry Blasting Operations; Permit Number M-2021-009 <br /> Dear [Homeowner]: <br /> Your home, shop or other manmade structure is located within the pre-blast survey area of the <br /> Young Ranch Resource Quarry. We will be taking precautions at our blasting operations and <br /> intend to prevent damage to your structure by following restrictions established by the mining <br /> permit and state and federal agencies. We are offering, in good faith, a pre-blast survey to every <br /> resident and owner of a structure within the pre-blast survey radius of one-half mile from the <br /> proposed disturbance boundaries for this mine permit. The pre-blast survey will record and <br /> document the physical condition of your structure prior to any blasting. The pre-blast surveyor <br /> will look at each room in your home and each outside wall and document the current condition <br /> using one or more of the following: a camera, sketches, drawings, narrative descriptions or a <br /> combination of methods. If your water supply is a well or spring, a water sample will be taken <br /> and an analysis of the untreated water will be included with the survey. The surveyor will also <br /> measure the water level in the well and the well production rate if the well can be accessed. <br /> Pre-blast survey request and waiver forms are included with this letter. If you would like to have <br /> a pre-blast survey performed at no cost to you, please complete the "request" section of the form. <br /> If you do not want a pre-blast survey, please fill out the "waiver" section of the form. A stamped <br /> addressed envelope is included for your convenience. If we do not receive the form back from <br /> you before, we will assume that you have declined a pre-blast survey. <br /> A copy of the completed pre-blast survey will be sent to the Colorado Department of <br /> Reclamation, Mining, and Safety (CDRMS) for their records. The operator will keep a copy and <br /> you will be provided a copy. If you disagree with any results determined by the survey, you <br /> should send a written explanation detailing the points of disagreement to the DRMS and the <br /> operator. Your letter explaining the disagreement will be made a part of the pre-blast survey file. <br /> If you expand or renovate your structure, another survey of the renovations/additions will be <br /> performed at your request. To request a survey of the addition or renovation, contact the <br /> CDRMS at(303) 866-3567 or send a written request to the operator. Your request should include <br /> your name, address and phone number with a short description detailing why a up-dated pre- <br /> blast survey is necessary. <br /> If you have any questions about the blasting, please contact me at [(XXX) XXX-XXXX.] <br /> Sincerely, <br /> [Operator Representative] <br />
