DocuSign Envelope ID:6BC32598-B737-464E-898A-81 F1 F7131339133
is 20 ft above the top of the Lloyd Sandstone member of the Mancos Formation. An intermediate
casing then extends from surface to a total depth of approximately 4,700 ft which corresponds to
the bottom of the Meeker Sandstone member of the Mancos Formation. The intermediate casing
will be perforated in the sandstone members of the Mancos Formation and any other promising
injection zones in the Mancos Formation. These are anticipated to the Lloyd, Duffy, Morapos, and
Meeker Sandstone members. The corresponding perforation intervals in the Mancos Formation
will be approximately 2,690- 2,735, 2,800—2,870, 3,495 -3,530, and 4,570-4,700 ft bgs.
Well Abandonment
The injection tubing and down hole apparatus will be removed from the well and any necessary
clean out conducted. The well fluid will be displaced with plugging gel. A cast iron bridge plug
(CIBP) will be set within the intermediate casing no more than 50 ft above the top of each
perforation zone and a minimum 20-ft cement plug will be set on top of the CIBP. This will be
done for each of the deeper perforation intervals (anticipated to be three) in the Mancos
Formation.
A CIBP will be set right above the top of the Lloyd Sandstone perforations at approximately 2,690
ft bgs. A minimum 200 ft cement plug will be placed above this CIBP. This plug will cover the
surface casing shoe and approximately 180 ft of the surface casing.
A CIBP will be set 200 ft bgs in the innermost casing string and cemented to the surface to isolate
the well bore from any surface fluid migration paths.
Attachment F. Financial Assurance
Colowyo will provide financial assurance upon reaching a mutually agreeable well abandonment
cost;this will be provided prior to final approval of permit application.
Attachment G. Site Security and Manifest Requirements(Commercial Wells Only)
Not applicable.
Attachment H. Aquifer Exemptions
No aquifer exemptions are currently requested. Aquifer exemption requirements will be assessed
based on site-specific water quality data when the well is drilled and background water chemistry
is determined.
Attachment I. Existing EPA Permits
Colowyo's EPA ID number is COD081474934. In Colorado,CDPHE reviews the USACOE Section 404
permits and provides a Section 401 certification that the mine complies with. The Colowyo Mine
currently does not have an active 401 permit. Colowyo does not have any of the other permits
listed on the Form 7520-6 instructions.
Colowyo has the following active State Permits:
15
