Well failures will result in well shut-in until the causes <br /> have been successfully determined and addressed. <br /> Spill/Release Response <br /> The well operator will comply with federal and state spill response/reporting requirements. If a <br /> spill or release were to occur,field personnel are instructed to: <br /> • Assess immediate danger or threat to health, safety, and welfare and contact emergency <br /> responders, if appropriate. <br /> • Locate and stop the source of the spill, if safe to do so. <br /> • Report the spill to supervisors and arrange for additional response support, including <br /> contact support. <br /> • Take whatever steps are necessary to prevent the spill from impacting surface water, <br /> public health, safety, and welfare. <br /> Monitoring <br /> Realtime well monitoring will be conducted as specified in Table D-3. <br /> Table D-3. The quarterly reports will include: <br /> • Monthly average, maximum and minimum values for injection pressure, flow rate and <br /> volume and annular pressure. <br /> • Water analysis reports including major anions and cations, pH, TDS, and any other <br /> constituents whose concentrations have a limit set in the permit. <br /> 13 <br />
