The following items are incomplete for <br /> the application to be processed: <br /> Financial Warranty <br /> 1. No financial warranty was included with the application form.A reclamation bond will <br /> be required for this permit. The Division will need to first conduct a site inspection to <br /> calculate the current reclamation liability in order to determine the required bond <br /> amount. The Division will notify both parties of the required bond amount as soon as <br /> possible. <br /> 2. An Affidavit of Authority form will also need to be submitted with the Financial <br /> Warranty forms. <br /> Application Form <br /> 3.
