Enclosed <br /> with this submittal are the following items. <br /> - Proof of posting signs at the mine entrance in accordance with Rule 1.6.2(b) <br /> - A table of contents, which lists the contents of the permit package <br /> - A checklist cross referencing the location within the permit application of each section of <br /> the Rule <br /> - Permit application form for Section 112d Designated Mining Operation <br /> - A check in the amount of$4,025 to cover the cost of an existing Section 112d(2)permit <br /> amendment <br /> - An Executive Summary of the proposed changes to the affected lands boundary in <br /> Governor Basin as well as a summary of Technical Revisions that have been submitted <br /> since the last permit amendment in 2015 <br /> - Exhibits A through U plus a geotechnical stability exhibit <br /> - Appendices 1 through 8, which include supporting documentation <br /> Once OSMI has received DRMS's determination of a complete application, a public notice will <br /> be published in the local newspaper for four consecutive weeks.
