Inc. (OSMI) would like to notify the Division of Reclamation. Mining and <br />Safety (DRMS) that sample results from the loss of containment incidents that occurred on <br />October 6"' and October 8`". 2021, have been received. <br />On October 6`". approximately 200 gallons of tailings overflowed from the filter feed tank. The <br />tailings left secondary containment but did not leave the mine site nor did the material impact <br />surface water. The spilled tailings were contained within 10 feet of containment and were <br />cleaned up and placed in the lined temporary tailings storage facility w ith other tailings to dry. <br />SPLP was performed on the spilled material and results compared to groundwater standards. All <br />constituents are within groundwater limits. <br />On October 8`", approximately 100 gallons of process water overflowed from the filter feed tank. <br />The process water left secondary containment but did not leave the mine site nor did the material <br />impact surface water. The spilled process water was contained within 15 feet of containment. <br />Waste rock outside of containment that was impacted by the process water was removed and <br />placed in the lined temporary tailings storage facility with other tailings to dry. SPLP was <br />performed on waste rock impacted by the spilled process water and results compared to <br />groundwater standards. All constituents from the leachate procedure are within groundwater <br />limits except pH with a result of 9.8 compared to an upper groundwater permit limit of 9. <br />Based on results of SPLP testing, no further cleanup action is needed. <br />OSN11 appreciates your attention to this matter.
