I November 2021 C-1981-044/Williams Fork Mines RAR <br />o Access roads around the site appeared in good repair with no ruts or standing water. <br />o Ditches appeared to be functioning adequately. <br />HYDROLOGIC BALANCE Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge <br />Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring <br />4.05.13; Drainage- Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream <br />Buffer Zones 4.05.18 <br />o Unless otherwise noted, pond banks and dams appeared stable at the time of inspection. <br />o None of the ponds held water at the time of inspection. <br />TOPSOIL RULE 4.06 <br />Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution <br />4.06.4 <br />o A series of small topsoil piles are scattered throughout the site. <br />o Piles appeared to be marked appropriately with t posts. <br />o All topsoil piles were vegetated. <br />BACKFILL and GRADING RULE 4.14 <br />o Backfilling and grading at the portal was not occurring at the time of inspection. <br />o A deep trench exists where the culvert at the portal was dug up, removed and disposed of. <br />RE VEGETATION - Rule 4.15 <br />General Requirements 4.15.1; Use of Species 4.15.2, Grazing 4.15.5 <br />o Reclamation and revegetation are progressing as permitted. <br />o Seeding that occurred in fall of 2019 and 2020 along the southern bottomlands of the <br />Williams Fork River exhibited many weeds. Grasses and other seeded vegetation that had <br />struggled in the dry summer were present throughout. The plan is to mow the weeds and <br />broadcast seed into the mowed mulch. <br />o Vegetation on the refuse pile (reclaimed in 2020), showed an abundance of weeds. <br />o The soil was saturated throughout the site and the vegetation appeared to be responding to <br />the recent rains after a hot, dry and dessicating summer. <br />INSPECTION, ENFORCEMENT and CIVIL PENALTIES RULE 5 <br />Inspections 5.02; Availability of Records <br />Permit documents were inspected.
