Describe Proposed or Completed Operation:Clearly state all pertinent details,including estimated starting date of any proposed work and approximate duration thereof.If <br /> the proposal is to deepen directionally or recomplete horizontally,give subsurface locations and measured and true vertical depths of all pertinent markers and zones.Attach <br /> the Bond under which the work will be performed or provide the Bond No.on file with BLM/BIA.Required subsequent reports must be filed within 30 days following <br /> completion of the involved operations.If the operation results in a multiple completion or recompletion in a new interval,a Form 3160-4 must be filed once testing has been <br /> completed.Final Abandonment Notices must be filed only after all requirements,including reclamation,have been completed and the operator has determined that the site <br /> is ready for final inspection.) <br /> All depths referenced to GL.The IRI-26-EX-2 well reached the end of its useful life and has been plugged and abandoned.A 3.875"bit and 4.5"casing <br /> scraper were tripped in and the well was cleaned out to a depth of 1,875'.The 4.5"casing was perforated at depths of 101',and 1110'.Both of the <br /> perforation intervals consist of four 0.38"holes at 90 degree phasing and with 20.5"of penetration.All cement plugs were set using the balance method. <br /> Plug 1 cement consisted of 18.0 bbis(78.75 sks)of 14.5 lb/gal Portland Type 1/11 LA cement and was pumped in three lifts,the first lift of 7.2 bbls from <br /> 1,835',the second lift of 7.2 bbls from 1,535',and the third lift of 3.6 bbls from 1,135'.Plug 1 was allowed to set 19.5 hours,after which the top of cement <br /> was tagged at 798'.The calculated volume of the plug 1 interval(1,875 to 798')was 17.18 bbls(96.46 cu ft)which meant that 4.8%excess(0.82 bbls) <br /> cement was pushed out the perforations between 1,842'and 1,868'feet and out the perforations at 1,110 into the adjacent casing annulus.Plug 2 cement <br /> consisted of 13.7 bbis(59.9 sks)of 14.5 lb/gal Portland Type 1/11 LA cement and was pumped in two lifts,the first lift of 7.2 bbls from 776,and the second <br /> lift of 6.5 bbls from 475'to surface.The cement fell back after pumping to 25'.The calculated volume of the plug 2 interval(798'to 25')was 12.33 bbls <br /> (69.2 cu ft)with which meant that 10%excess(1.37 bbis)cement was used to plug the upper cased and annular portion of the well.The well was dug out <br /> at surface and the casing was cut off 3 feet below GL.The 4.5"casing was topped off from 25'to surface with 2 sks(0.6 bbls)of 15 ppg Portland Type 1/11 <br /> cement.A total of 32.3 bbls(140.65 sks)of cement was used to P&A the well.A steel cap with the well name was welded onto the 7.0"conductor casing <br /> and buried on 10/19/2021.
