0 COLORADO
Division of Reclamation,
Mining and Safety
Department of Natural Resources
November 1,2021
Eric Leigh
Monarch Mountain Minerals and Aggregates,LLC
2660 West 64'Ave
Denver,CO 80221
Re: Reason to Believe a Violation Exists and Notice of Board Hearing;
Lily Mines; Permit M-1987-028
Mr. Leigh:
On June 10, 2021,the Division of Reclamation Mining and Safety(Division)notified you that the
required geotechnical reporting information,which is required annually, had not been received since
2018.Annually submitting a geotechnical report regarding construction and stability of highwalls and
benches at the site is a condition of your pen-nit.
