BOX 187 <br />Craig, CO 81626 <br />29 September 2021 <br />Re: Trapper Mining, Inc., Permit C1981010 <br />2021 Quarter 3 Horse Gulch Fill Inspection Report <br />Dear Ms. Perkins: <br />The Colorado Division of Reclamation, Mining, and Safety (CDRMS) has reviewed and <br />found the above referenced report to be in general compliance with the following CDRMS <br />regulations: <br />4.09.1(11(a) Inspected at least quarterly by a registered engineer, or other qualified <br />professional specialist in construction of earth and rockfall embankments under the direction <br />of a qualified engineer throughout the construction. <br />The certified report covers the third quarter monitoring period from July through September of <br />2021, and the inspection was conducted by yourself, a certified engineer on 14 September <br />2021. <br />4.09.1(11)(b) Provide to the Division a certified report within 2 weeks after each inspection <br />that the fill has been constructed as specified in the design approved by the Division. The <br />report shall include a description of any appearances of instability, structural weakness, and <br />other hazardous conditions. <br />The Division received the report via electronic transmission on 22 September 2021. No <br />instability, structural weakness, or other hazardous conditions were noted during this <br />inspection of the fill. The area where cracks were detected on the top south portion of the fill <br />during the first quarter of 2013 was inspected and no additional cracking was observed. Other <br />areas where differential settling has been observed in the past were also inspected.
